You, or your physician, have arranged a new patient consultation with Dr. Fraser.
We are happy you will be coming to see us and will do everything we can to make your first visit with us go smoothly. Please take time to read the following set of instructions for new patients. Following these simple instructions step-by step to ensure we have the proper medical information on your condition to thoroughly evaluate your condition, ensure that insurance coverage will be provided for you, and allow us to send all relevant medical information to your family doctor.
Before Your First Visit - Patients who do not have their paperwork completed at the time of their appointment will be rescheduled.
- Discuss your pending appointment with your family doctor.
- If your doctor did not refer you, please ask them to send us any relevant medical information they may have on you arthritic condition prior to your appointment.
- Download and complete the New Patient Form. Bring this with you to your appointment.
- All new patients must complete a medical questionnaire detailing their medical and arthritis-related history.
- It is designed to take no more than 20 minutes to complete.
- Read the Notice of Privacy Practices - This contains detailed information concerning what Coastal Arthritis & Rheumatism does with your protected health information and what your rights are concerning your protected health information.
- Download and sign the HIPPA Patient Authorization Form. Bring this with you to your appointment.
- This form is necessary in order to share your private health information with family members you designate.
Your First Visit
Please arrive for your appointment at least 30 minutes prior to ensure all required paperwork has been completed.
Please bring all insurance cards so we may make copies for our files.
Please bring pertinent x-rays, lab report from your referring doctor for us to review.
After Your Visit
All medical reports will be sent to your referring physician unless otherwise authorized by you.
